Safety and Health

Our job is not an easy one, but it should be a safe one. Reporting all violent, serious, and near-miss incidents is an important part of the process to ensure that all SOTA members have a safe and healthy workplace.  
How to Report 
All reporting happens online via Employee Connect- no paper forms are accepted
Inform your administrator if an accident, injury and/or a violent incident has taken place.
Fill out the online form through Employee Connect if you have experienced, or been a witness to, an accident, serious incident,  violent incident, or a near miss. 
 
1. Log in to Employee Connect and complete an Incident report.
  • Go to the "INCIDENT" tab
  • Click on "Add a New Incident" box
  • Fill in the information
  • Click "Save" box

2. If you are unable to work, you MUST:  
  • Enter a leave request in Employee Connect – using Illness/Medical as the leave type and Workplace Injury for the absence reason.
 
3. If you are making a claim for reimbursement of loss of sick leave due to injury on the job, you
    must apply by email to the Assistant Superintendent – Personnel.
