Please fill out the online form through Employee Connect if you have experienced or been a witness to an accident, serious incident or violent incident.
1. Log in to Employee Connect and complete an Accident/Incident Report.
2. If you are unable to work, you MUST: Enter a leave request in Employee Connect – using
Illness/Medical as the leave type and Workplace Injury for the absence reason.
3. If you are making a claim for reimbursement of loss of sick leave due to injury on the job, you
must apply by email to the Assistant Superintendent – Personnel, in accordance with Article
6.11.7 of the S.O.T.A. Collective Agreement, which reads as follows:
“No teacher shall suffer a loss of sick leave due to injuries sustained on the job and/or
school sponsored extra-curricular activities and related supervision. The teacher
injured shall be placed on sick leave, short term and/or long-term disability insurance
until he/she is able to return to work. When the teacher returns to work, the Board
will re-instate his/her sick leave entitlement accumulated prior to the injury."
