Our job is not an easy one, but it should be a safe one. Reporting all violent, serious, and near-miss incidents is an important part of the process to ensure that all SOTA members have a safe and healthy workplace.
How to Report
All reporting happens online via Employee Connect- no paper forms are accepted
Inform your administrator if an accident, injury and/or a violent incident has taken place.

Fill out the online form through Employee Connect if you have experienced, or been a witness to, an accident, serious incident, violent incident, or a near miss.
1. Log in to Employee Connect and complete an Incident report.

must apply by email to the Assistant Superintendent – Personnel.

- Click on "Add a New Incident" box
- Fill in the information
- Click "Save" box

- Enter a leave request in Employee Connect – using Illness/Medical as the leave type and Workplace Injury for the absence reason.
3. If you are making a claim for reimbursement of loss of sick leave due to injury on the job, you
